Thank you for your thoughtful intention to make a gift of securities to the Pen Bay Healthcare family of services, which includes the Penobscot Bay Medical Center, Kno-Wal-Lin Home Care and Hospice, Anderson Inn at Quarry Hill, the Knox Center for Long-Term Care and Mid-Coast Mental Health Center.

 The instructions for making an electronic transfer of securities or for presenting physical stock certificates are listed below. TRANSFER OF STOCK CERTIFICATES

If you have in your possession the physical stock certificates, you may bring them to UBS  Financial Services at 30 School Street, Second Floor, Rockland, Maine, 04841, to be deposited into the Pen Bay Healthcare account.
