Access Services provides a single point of contact for callers requesting service or looking for information about a behavioral health outpatient program.
 
The Mid-Coast Mental Health team is committed to providing a friendly, informative and timely response. To request an outpatient-based behavioral health service, please call 701-4446 or 800-540-2072.

Prior to seeing a treatment provider, you will have an  administrative intake appointment.  You will need to bring with you to your first appointment the following:

• Insurance card/Medicaid card
• Verification of income if uninsured (pay stub, income tax form)
• Pre-authorization number for your first visit, if you have managed care.  This is obtained by calling the number on the back of your insurance card.
• If you are under the age of 18, you will need to come with a parent or guardian who can sign the paperwork for you.

Other functions of the intake process are to:

• Inform you of your rights and responsibilities.

• Obtain your consent for treatment.
• Set up an agreement about payment terms.  This may be through  insurance or on your own.  We offer a sliding scale fee arrangement for those clients who pay for services themselves.
